1) Screw the thumbscrew at the top to lower the carrier with the barbell all the way down to the bottom
2) Insert your nipple through the 3/4" diameter round hole.
3) Insert the barbell through the carrier as well as your nipple, and screw the ball on the end of the barbell to secure it in place.
4) Then turn the thumbscrew on top clockwise to raise the barbell and apply tension.
5) Start slowly and work your way up. Do not put too much tension on at one time because you do not want to tear your piercing.
Over time a 2" stretch on your nipples is possible though.
